Florida PIRC at USF is committed to continuous improvement. The information acquired from these Evaluation Response forms provides us with the necessary feedback to better serve parents and educators. As a condition of using a Florida PIRC at USF workshop, we ask that you please have your workshop participants anonymously complete the PIRC evaluation forms. Please mail or fax copies of the completed evaluation forms along with a copy of your workshop sign-in form to:
